The age selected for this analysis of child abuse are toddlers, children aged between 0 and three years. They face the highest cases of child abuse mainly due to many of their caregivers not knowing how to talk care of them (Cook et al., 2017). At this age, the most common type of abuse they face is neglect as the parents don’t provide the proper food and nutritional affection they require. Moreover, the neglect extends to medical and other necessities that are vital for their proper development. Some of the most common signs that nurses should not are the child’s constant unrest, nutritional deficiency and parent’s lack of interest on the child’s overall health (Cook et al., 2017). A common condition that is seen in toddlers is the shaken baby syndrome a form of abusive head trauma caused by forceful shaking of the infant or toddler. With the recent rise in immigrants from Asia a common Southeast Asia, a cultural practice that has been misinterpreted by the medical community is “Cao gio” (Tanner, Catanese, Lew, & Rapkiewicz, 2016). It is a dermabrasion therapy technique that is used for various conditions such as relief of pain and nausea. In the majority of the states within the U.S. inclusive of this state, the reporting mechanisms involve the first the separation of the child from the caregiver (Tanner, Catanese, Lew, & Rapkiewicz, 2016). This is simultaneously followed with the notifications of the authorities that include the police as well as Child Protective Services. The process has to be swift to ensure maximum attention is given to the child, moreover, by contacting the police the abusers will be apprehended. Finally, the Child Protective Services will provide necessary protection to the child as well as a new home. This procedure has been proven effective in many occasions and is yet to be challenged.

 Three advocates which I am choosing in regards to bringing about change to public policy are Florence Nightingale, Hildegard Peplau and Dorothea Orem.  Each of these leaders has brought about some form of change to nursing in their own way.  Florence Nightingale focused on the importance of the observational skills of nurses and how the environment impacted health.  Hildegard Peplau was more focused on ways to help people stay well, rather than treating them once they were ill (Nickitas, Middaugh & Aries, 2016).  Dorothea Orem’s model was focused on the individual caring for themselves, rather than being cared for by others.  Orem’s model for changed is composed of three components to make up her model; the theory of self-care; theory of self-care deficit, and theory of nursing system (Petiprin, 2016).  Because of the rapid change and development of healthcare and medicine, it is important that nurses are also moving forward with these changes.  In order for health care and public policy change to be effective, nurses need to able to influence the formation of health care policies rather just the implementation of the policy.  The influence of the nurse upon health policy and public policy reform is important as it protects the interest of the patient and their safety, increases the quality of care, and helps to facilitate access needed to the required resources in order to promote quality in health care (Arabi, Rafil, Cheraghi, & Ghiyasvandian, 2014).  While nurses are used to serving as an advocate for their patients, and putting the patient’s needs and wants first, it is also important that nurses be advocates for each other in the professional aspect of healthcare.  As issues with health care related reform are brought about and evolve, issues sometimes cause stress and fatigue upon the healthcare system and its workers.  With this in mind, it is imperative that we, as nurses continue to recognize commitment to a common goal and being willing to make themselves available for the advancement and promotion of the growth of nursing younger colleagues (Huber, 2015).

There was a cross-sectional study conducted in 2013, with the use of data from 21 University Health System Consortium hospitals,that analyzed the association between RN education and patient outcomes. The results from this study showed that Hospitals with a higher percentage of RNs with baccalaureate or higher degrees had lower congestiveheart failure mortality, decubitus ulcers, failureto rescue, and postoperative deep vein thrombosis or pulmonary embolism and shorter length of stay. This particular article stated that how studies dating back to 2002, linked the percentage of RNs in a hospital with baccalaureate degrees to decreased patient mortality (in-hospital and 30-day mortality, failure to rescue) However, other studies have not found significant relationships between mortality and nursing education. It further mentinoed how the few studies that examined the impact of baccalaureate education on other patient outcomes, particularly those that are considered sensitive to nursing care, did not find beneficial effects. This study also mentioned how the ducation of RNs did not affect hospital-acquired infections once other characteristics were controlled, whereas nurse staffing levels did (Blegen, Goode, et al, 2013).

Looking back on real life experiences, one thing I have observed is that some of the BSN prepared nurses that I have worked with were more skilled at critical thinking that the ADNs which now I can see why after being in this program which requires much more research. The BSN prepared Nurses are more detailed oriented in assessments and charting as well.

The American Cancer Society offers a multitude of resources
to cancer patients all over the United States. The offer everything from
education and support groups to research and financial assistance. The American
Cancer Society is community-based volunteer health organization that
specializes in cancer research, treatments, as well as helping not just the
patients but the families as well (ACS, 2017). ACS has a wide variety of
options for services and recognizes that each person’s situation is unique and
different but they will have reliable options available to them (2017). They
also offer free wigs and lodging at centers if the patients treatment is too
far from their own house to make the commute feasible (ACS, 2017).

 

If I had a friend that was recently diagnosed with cancer I
would recommend them going to the American Cancer Society and formulating a
specialized treatment plan with the providers. Once a treatment plan is in
place then the people at ACS can help to determine how they can best meet his/her
needs.

 

Factors that contribute to the yearly incidence and
mortality rates of cancers in Americans are: diet, exercise, stress, tobacco
use, infectious agents, and environmental risk factors (ACS, 2017). Americans
tend to consume higher fat and processed food diets than most other countries
with less exercise than other countries as well too (ACS, 2017).  According to the American Cancer Society “An estimated 20% of all
cancers diagnosed in the US are caused by a combination of excess body weight, physical
inactivity, excess alcohol consumption, and poor nutrition, and thus could also
be prevented (2017, p. 1)”. Mortality rates for Americans with cancer peaked in
1991 to 215 cases per 100,000 people then dropped significantly in 2017 to
100,000 – this equates to an approximate 25% decline (ACS, 2017). Part of the
decline in mortality rates are linked to less Americans smoking and being more
educated on the risks that lead to cancer (ACS, 2017). One change in policy
that has helped with these rates is the early screening and detection that is
being done in the US (ACS, 2017).

 

“The American Cancer Society has a long history of finding answers
to critical questions about lung cancer – what causes it; how can it be
prevented, detected, and treated successfully; and how lung cancer patients’
quality of life can be improved. These efforts have helped reduce lung cancer
death rates in men by 36% over the past two decades and in women by 11% since
2002. Despite this progress, lung cancer is still the leading cause of
cancer death in the U.S. (www.cancer.org,
nd, p. 1)”. In
2014 The American Cancer Society began a partnership with Stand Up To Cancer to
help fund additional research for lung cancer and developing new therapies for
lung cancer (ACS, nd). This research will have a significant impact on
prevention and treatment of lung cancer worldwide. While it has been noted that
smoking has declined this decline has been seen more significantly in younger
women. Older women continue to smoke and the rates for lung cancer in this
population has increased (ACS, nd). Lung cancer is also caused by environmental
exposures and performing research in this area will also help lead to a
reduction in lung cancer rates in the future (ACS, nd).

The Patient Protection and Affordable Care Act clearly represents a movement toward much needed, comprehensive and meaning reform for our nation’s health care system. The Nursing Workforce Development programs will recruit new nurses, promote career advancement and improve patient care.

 These programs are also used to direct nurses in areas of greatest needs.  Public Health Workforce Loan Repayment Program assures an adequate supply of public health professionals to eliminate workforce shortages in public health agencies. In this program, HHS would repay up to 1/3 of loans incurred by a public health or health profession student in exchange for that student’s agreement to accept employment with a public health agency for 3 years.

 This is a great program. It could possibly help decrease the shortage of nurses. This will definitely get the attention of a lot of new nurses. I think it’s a great idea to offer the nurses something valuable in exchange for their agreement to work for the company. This can bring aboard a lot of nurses. The more nurses a facility has, patient outcomes will improve. It will also decrease the workload on nurses. When the workload is decreased, nursing turnover rates will also decrease. Nurses will have more time to spend with their patients and educate them on their disease process or prevention tips. Patients will have an opportunity to ask questions without feeling like they are holding up the nurse. The patients will also be more ready to learn.  

Nurse-Managed Health Centers authorizes grants that provide comprehensive primary care or wellness service without regard to income or insurance status for patients. These centers must provide care to underserved or vulnerable populations and be associated with an academic department of nursing, qualified health center or independent nonprofit health or social service agency.  These centers can help change lives.

 People that cant afford health care can be seen. Children living in poverty can be seen and vaccinated to prevent certain diseases. They wouldn’t have to worry about avoiding going to see a doctor because of their insurance or no insurance at all. This can have a big impact on nursing. Nurses can educate these populations on disease prevention and management.

Vaccine Controversies

There is no doubt that the topic of vaccine has received numerous fears because of its below 100% effectiveness, among other reasons. On the other hand, the campaign for vaccine has been strongly recommended because of the lives saved through it and the diseases it has prevented such as polio and small pox. In this discussion, I will attempt to look at the two perspectives with a view of shading light to what is the most important, severe and useful to mankind regarding vaccine controversies in the world today.

Since vaccine was introduced, there have been rivalry by religion on unfounded facts. Owing in mind many unvaccinated children have passed away, religions remain firm with their Simon says hence nations give in to their negative ideas. On the question of safety versus sanity, the cell has some viral vaccines which has to be gotten from humans. Rubella vaccine is derived from tissues taken therapeutic abortions performed in 1960s.

In Kerala a magazine had an article of certain minor post-vaccine adverse events causing death. A boycott of vaccine was called and vaccines called “agents of death”. The respond to that was predictable as there was a heated debate on television channels. Later the article was removed since there was opposition lead by Indian Medical Association but by then many had already accepted the vaccines should be eradicated. Before this, in Malappuram district of India, religion groups said that vaccines was targeted to demolish a certain religion. As a result 100% of children in that district have been vaccinated but only 74% are fully protected. A campaign is needed being led by IMA (Indian Medical Association) to stop the spread of myths against vaccines (Vasudevan, 2016). 

In conclusion, vaccines save lives but the need to draw a line between what is right or acceptable and what are wrong calls for a lot of compromise from all parties. The examples above show that more research also need to be undertaken to find better ways to cope with allergies from vaccines so as to improve on survival rate after vaccination.
